Doubt about forms and save information in database

Hello everyone! I want to include a form in my application where the user fills in the fields, selects alternatives (example: checkbox) and uploads images, and this information is stored in a database.

Is all this possible in Mobincube?

How can you build a form in the application and send the information to a database?
