Need a Ride Share app

I'm looking for the most basic ride share service app possible. On my website, I'm able to have customers fill out a form: name, address, passengers, destination. Four fields that's it. Then when they press "request ride," a pay feature pops up that allows them to "donate" the suggested amount. On the driver's end that request is seen by them on their email account. So I am not looking for all the fancy stuff that you see like the live feature, the different type of cars, the choose an address by using a map. I just want this form, a nice design and that's it. Something this simple is all I want, and so that's what I'm looking for. I'm not looking to spend thousands of dollars on an app like this if the same feature exists on a $3 website I created.

Hello RidePoolIU,

at the moment the only form available is a basic one where users can send some info through the app.

If you want a more complex form you can use a web screen and embedd an external form that you can find in internet. There are many online form builder. that you can use to obtain an url to insert into the web screen.

At the moment those are the 2 options available.
